Each participant must complete the following:
-Adult or Student application (Please provide your date of birth and write your name as shown on your passport or passport application)
-Volunteer Pledge
-Release and Waiver of Liability Form
-Medical Forms
-Scholarship form (only if in need of financial assistance)
